We encourage all applicants to use this interactive form, which can be filled in online. This is the best way for us to get your information as it connects directly to our database.

Please make sure your application form is filled out completely, in the format requested, with all supporting documentation attached. Please keep your answer to the stated word limit. 

Unless already provided to the Ministry of Social Development, all applications must include the last two years’ audited financial statements, or similar.

Apply using a printed application form

Only use a printed form if you cannot access the online form.

Download the printed Application Form in the pdf format. Print this form on single-sided A4 paper, complete in full, and send in unbound format to the address below. Check the form is complete with all necessary supporting documentation attached.

Please call 0800 777 100 if you are unable to access either the online or pdf application form and request a form be sent to you. These can be sent in other formats, such as a word document, for those with accessibility issues.
